AFFORDABLE CONNECTIVITY PROGRAM DISCLOSURES
The Affordable Connectivity Program (“ACP Program”) is a government program that reduces the cost of your broadband Internet access service bill. The NCKCN Service provides download speeds of up to 25 Mbps down and 3 Mbps up. There is no data cap on your plan.
The ACP Program is a federal government benefit program operated by the Federal Communications Commission (“FCC”). Upon the conclusion of the ACP Program, as announced by the FCC, or if you are no longer eligible for the ACP benefit, you will be subject to NCKCN’s undiscounted rates and general terms and conditions if you continue to receive services from NCKCN. If you were an existing NCKCN customer at the time you joined the ACP Program you will be subject to your NCKCN’s rates and Subscriber Agreement in effect prior to your enrollment in the ACP Program. Your monthly pricing will revert to the pricing in your agreement prior to the ACP Program, and all terms and conditions of that agreement, including any applicable Early Termination Fees, will be effective. The ACP benefit is non-transferable and limited to one discount per household.
You may obtain broadband service supported by the ACP program from any participating provider of your choosing.
You may transfer your ACP Program benefit to another provider at any time.

HAVING READ THESE DISCLOSURES AND BY APPLYING FOR OR ESTABLISHING AN ACCOUNT WITH NCKCN YOU ARE CONSENTING TO THE APPLICATION OF YOUR ACP PROGRAM BENEFIT TO YOUR SERVICE RECEIVED FROM NCKCN. IF YOU WERE AN EXISTING NCKCN SUBSCRIBER AT THE TIME OF OUR ENROLLMENT IN THE ACP PROGRAM YOU ACKNOWLEDGE AND CONSENT THAT UPON TERMINATION OF THE ACP PROGRAM, AS ANNOUNCED BY THE FCC, OR IF YOU ARE NO LONGER ELIGIBLE FOR THE ACP BENEFIT, YOUR SERVICE PRICING WILL REVERT TO YOUR PREVIOUS PRICING PRIOR TO THE ACP PROGRAM FOR THE REQUIRED REMAINING TERM OF YOUR ORIGINAL AGREEMENT.
